A Clinical Evidence based approach to improve quality and safety of care through measurement & governance
Outline: Barnsley Hospital measure and assess quality of pressure ulcer prevention and falls prevention through Perfect Ward audit tools. Perfect Ward evaluates how clinical areas are implementing the best practice interventions respective to the prevention of inpatient falls & pressure ulcer prevention. Audit findings identify specific care interventions for improvement for individual teams, which coincidentally have been identified in RCAs. This approach allows teams to identify potential lapses in care before harm has occurred. Applying these Perfect Ward Audits across all inpatient areas allows for trust wide benchmarking and driving standards of care higher in line with best practice. Since its inception in January 2020, we have been able to evidence an improvement in standards of care through perfect ward. Our focus is to educate and measure doing the right things for patients rather than focusing on measuring the harms.
Jonathan Hazan - Chair - Patient Safety Learning
Since retiring as chief executive of Datix in 2016, Jonathan has worked with a number of successful healthcare and IT businesses. He founded the charity Patient Safety Learning, which he chairs and is a trustee of AvMA. Jonathan is an advisor to Perfect Ward and chairs Hotel Radio, a music livestreaming company.
Gavin Portier - Head of Nursing Quality, Barnsley Hospital NHS Foundation Trust
Began his nursing career in Sydney Australia in 1994 and came to Sheffield in 2000. He has a clinical background in critical care since 1999 and recently returned to ICU in April to May in response to COVID19. Gavin has held the following nursing leadership roles; Registered Manager and Clinical lead for complex enhanced dementia, Service Manager for Community CAMHS, Head of Quality Compliance & Assurance and Head of Nursing Quality. His passion for organisational behaviour and quality improvement has enabled him to use Perfect Ward creatively to improve care and the governance structures that surround it.
